COMMUNITY

PATHWAYS

A network for parents, grandparents and carers with dependents of any age who have special needs including emotional and physical disabilities and neurodiversity (SEND).

We are a community led, voluntary support network ensuring no one in our community feels isolated or unable to cope.

WHO WE ARE

We support individuals and families with any additional needs from pregnancy and birth through to adulthood and end of life. We aim to connect people to the right help at the right time, offering trusted signposting, shared experience, advocacy and guidance from experts.

From drop ins at The Zone to coffee mornings, group chats and private consultations we want to build a compassionate Community Pathway navigating disability and additional needs from birth to end of life.

We can’t replace SENDco’s in school or health/educational professionals but we can support, guide, advise and advocate. We can help with completing online contact forms, arrange meetings with relevant personnel ‘Champion/Supporter’, facilitate initial meetings/conversations to discuss concerns and other possible areas of support, signposting, actions agreed and future meetings if appropriate.
